EMPLOYEE SELF ASSESSMENT FORM

INTRODUCTION

This form is completed by the employee in a performance appraisal process. The employee is requested to complete a self evaluation in which the employee is required to assess him or herself in a predetermined list of areas such as work related accomplishments and/or contributions for the current performance cycle.

EXTRACT SAMPLE OF FORM

Are there any aspects of your work where you have not done as well as hoped and why? (Give examples). What things could be done to improve on these aspects on your part, your supervisor’s, and the department/organisation?

List any training and/or developmental activities that would help you improve your work performance or enhance your career objectives.
