EMPLOYEE CHANGE OF DETAILS REQUEST FORM

Any any organisation, things change. Employees are transferred from one department to another or a job status is changed from part time to full time or an employee gets married and his/her marital status needs to be updated on the company systems. The Employee Change Of Details Request Form does just that. It regulates any such job related and/or personal changes which need to be updated. The CCMA and Labour Court furthermore places the onus on an employee to constantly keep the employer informed on items such as last known address and this form can be used to capture any such changes.
